It's a court requisition sent to the police station asking them to confirm whether an accused who hasn't appeared in court has in fact died, before the court proceeds further or considers abating the case against them.
Why does it print two copies?
One copy stays with the court record and the other goes to the police station being asked to submit the report — printing both together with a cut line avoids a separate reprint for service.
What happens if the police don't respond by the due date?
